[[ Probably not. We have to remember, and be completely fair;  Itachi’s extreme attachment to Sasuke was a result of what he faced when he was four plus having a baby brother born a year later. Itachi sees him as someone to protect. Someone innocent, not tainted, who deserves to grow up not seeing or going through what Itachi did. Their bond is circumstantial. If they met as adults and GSA did not occur, they might be acquaintances, or friends, but likely not lovers. ]]

under what circumstances can itasasu happen healthily

[[ Both Itachi and Sasuke must be separated from each other until well into adulthood. 

When they are both adults, they can re-unite, and a phenomenon known as genetic sexual attraction will most likely occur. They will develop feelings for each other. Whether they find out they’re brothers or not is up to the author, I think. Some people do, some don’t. 

Re-uniting Itachi and Sasuke as adults gets rid of the problems they would have in a canon timeline, IE the abuse, manipulation, torture, and so on. That isn’t to say their relationship  can’t be unhealthy in some way, as any form of relationship has a risk, but it wouldn’t be mutually abusive like their canon dynamic is. ]]

No I’m purely talking about Itachi in terms of his social status within the clan.

[[ Itachi still wouldn’t be wed to a ~high ranking~ female. There’s literally no material within canon nor the novels that supports this idea. They don’t have any customs like that. Fugaku nor anyone else ever suggested Itachi marry the strongest girl in the clan.

Sure, they want their own children to produce children with Sharingan. But the days of producing as many kids as you could and seeing which survived into adulthood and were the strongest (Tajima’s era) are long over. Like I said, they’re not that serious about producing powerful offspring, but they do prefer kids who have the Sharingan. There were a number of Uchiha in the clan who didn’t have the Sharingan. 

Fugaku knew Izumi liked him. That was enough for him. Enough to move him to tears. Izumi isn’t special, she isn’t high rank, she’s not “below his league” or whatever. She’s more than enough. ]]
